Princeton Longevity Center (PLC) is the original, independent executive health and preventive medicine center in the United States with nearly 25 years experience in comprehensive preventive and executive health exams.
PLC is physician-led and prevention-first, delivering a deeply comprehensive, one-day integrated healthspan evaluation that examines the whole person. From cardiovascular and metabolic health to cancer risk, brain and cognitive function, musculoskeletal health, lifestyle, and genetics, PLC looks beyond isolated organs or symptoms to understand long-term health risk and longevity. Every recommendation is evidence-based, and tailored exclusively to the individual’s goals.
Many modern longevity clinics focus narrowly on “anti-aging” components like hormone optimization, IV therapies, a single imaging scan, or a subscription model tied to ongoing services. Others operate within large, hospital-based health systems, where preventive longevity care is often a secondary offering rather than the primary clinical focus,
Princeton Longevity Center takes a physician-led, preventive-first approach. PLC is solely dedicated to preventive and executive medicine, combining advanced diagnostics, board-certified physician interpretation, deep longitudinal risk assessment, and a structured, actionable care plan.
PLC is not trend-driven or focused on selling services It is outcome-focused, built on decades of clinical experience in academic preventive and executive medicine, with one goal: identifying risk earlier and guiding meaningful, personalized prevention.

PLC is prevention-focused. The goal is to identify disease risk and physiologic changes before symptoms appear, when intervention is most effective. While treatment recommendations are part of every personalized wellness roadmap, PLC’s core mission is early detection, risk reduction, and lifespan and healthspan optimization.
At Princeton Longevity Center, the Comprehensive Exam and Executive Physical are the same in components, elective options, and exam packages. The difference between the two exams is only financial responsibility and not the quality, depth, or scope of care.
A typical executive physical elsewhere may include basic labs, an extended physician visit, and limited testing. At PLC, both a Comprehensive Exam and an Executive Physical deliver the same deeply comprehensive evaluation.
The distinction between the two comes down to payment structure. An Executive Physical is generally sponsored by an employer. An employer-sponsored plan will include the Comprehensive Exam as the foundation to the program, and the employer may also choose to cover additional elective items, packages, or provide a credit towards optional components. A typical “Comprehensive Exam” is a self-paid program initiated by the patient directly.
Regardless of financial responsibility, the goal is the same: to provide unparalleled access to physicians, testing, and expertise not available at a typical doctor’s office.
Princeton Longevity Center does offer Direct Primary Care in limited locations as an elective program to your Comprehensive Exam. When you select this option, your Princeton Longevity Physician will provide year-round care and guidance for both your preventive and typical primary care. Direct Primary Care offers continuous health services, acting as the first point of contact in the healthcare system. This may also cover management of chronic conditions (diabetes, hypertension), treatment of acute illnesses, and coordination with specialists.
With participation in a Comprehensive Exam only, Princeton Longevity Center does not replace your existing primary care physician. Instead, PLC serves as a high-level diagnostic and strategic health partner—working alongside your primary care doctor and specialists to support smarter, more proactive, preventive care. We are happy to provide your personal Primary Care Physician with a copy of your testing and recommendations for continuity of care. You will also leave with and be emailed a complete set of your reports, test results, and you will be provided with a disc with your images.
Founded in 2002 by Dr. David Fein, PLC has been operating for over two decades, serving executives, professionals, and health-conscious individuals from across the country. The longevity in the preventive medicine space reflects trust, outcomes, and clinical credibility.

Cardiovascular risk is assessed through a comprehensive, prevention-focused framework that integrates non-contrast cardiac imaging, medical history, physical examination, advanced laboratory markers, and personalized risk profiling. Based on individual goals and program selection, patients may choose from a range of elective cardiovascular assessments designed to identify early atherosclerosis and cardiac risk before symptoms develop.
These may include advanced Cardiac CT Angiography, Carotid CT Angiography, Full Body Vascular Imaging, advanced lipid analysis, metabolic and inflammatory markers, functional testing, and direct or calculated measurement of VO2max. Findings are reviewed by PLC physicians in context, helping patients understand their risk and explore appropriate next steps.
Princeton Longevity Center evaluates cancer risk through a layered, prevention-focused approach.
As part of the core PLC evaluation, patients receive a foundational cancer risk assessment, which may include a physician skin examination, review of medical history, lifestyle factors, family history, and relevant laboratory markers.
For patients who wish to go deeper, elective cancer-focused testing options are available. These may include advanced blood-based screening tools and genetic testing to assess inherited cancer risk.
For those seeking the most comprehensive level of cancer risk, PLC offers the CancerCheck Program, an in-depth, elective option that integrates an enhanced full body scan, blood-based multi-cancer detection screening, genetic testing for inherited risk all within a structured, physician-guided framework.
Princeton Longevity Center offers elective options for assessing brain and cognitive health as part of a personalized longevity strategy. These optional assessments are designed for individuals who want deeper insight into memory, thinking skills, neurological risk factors, and early signs of cognitive change.
For patients who choose this level of evaluation, PLC’s Memory & Cognitive Health Program provides a structured, physician-guided approach using targeted cognitive and neurologic assessments, with results reviewed in context and aligned to individual goals.
Genetic and advanced biomarker testing is offered as an elective upgrade option for patients who want deeper insight into inherited risk and underlying physiologic patterns. These optional tests are designed to complement the core PLC evaluation and support more personalized prevention and longevity planning.
For patients who choose this level of assessment, PLC’s Genetic Insight Package integrates genetic analysis and advanced biomarker panels within a physician-guided framework, helping translate results into meaningful, actionable understanding.
Every Princeton Longevity Center evaluation is highly personalized, with no two programs designed the same. Testing and recommendations are tailored to age, sex, family history, lifestyle, goals, and risk profile. PLC offers a wide range of Elective Options to further customize your exam for you:
PLC evaluations are a customizable premium medical service, with competitive pricing that reflects the depth of diagnostics, physician time, and advanced testing involved. Exact pricing is easily provided, but varies based on how you customize your exam. A personal Patient Care Coordinator (PCC) is provided when you contact PLC by phone, email, or chat. Your PCC will assist with building an evaluation that suits your individual needs and budget, and an exact price will be provided, ensuring full transparency and no surprises. Discounted corporate, spouse, and family rates are also available, so please feel free to ask.
Princeton Longevity Center is not in network with commercial insurance plans and does not accept Medicare. This allows our physicians to spend unrestricted time with patients and deliver the depth of evaluation and clinical insight required for truly extensive and preventive care.
While PLC operates primarily as a self-pay medical service, some components of your evaluation may be eligible for insurance reimbursement if you have out of network benefits with your private health insurance. Our Patient Care Coordinators are available to help answer questions and provide guidance based on your specific situation. Patients with Medicare as their primary insurance will have no coverage with no possibility of reimbursement.
Yes – Many services may be eligible for FSA or HSA reimbursement. Patients are encouraged to check with their plan administrator.
Princeton Longevity Center is designed for individuals seeking a proactive, prevention-focused approach to long-term health – not for diagnosing active or unexplained medical symptoms.
For patients focused on early risk identification and strategic health planning, PLC offers value by integrating preventive evaluation, advanced diagnostics, and physician interpretation into a single, coordinated experience. This approach provides clarity around risk, helps avoid unnecessary or duplicative testing over time, and supports informed decisions about when specialist care may be appropriate.
Once testing is complete, results are reviewed the same day with your board-certified physician and integrated into a clear, prioritized action plan.

At Princeton Longevity Center, longevity medicine means extending healthspan, the years of life lived with strength, cognitive clarity, quality, and vitality. PLC’s approach focuses on identifying risk early, preserving function, and preventing disease progression long before symptoms appear, rather than reacting after illness develops.
PLC locations are easily accessible from major metropolitan areas, and many patients travel nationally to receive care through PLC’s comprehensive, preventive model. Don’t hesitate to ask our Patient Care Coordinators for travel assistance – we’re happy to help!
Yes. Lifespan adds years to life but Healthspan adds life to years. So, while living longer (lifespan) is often a goal for many patients, healthspan (quality of life) is a foundational pillar of PLC’s philosophy and clinical model. With a healthspan focus, PLC aims to increase the number of years lived (lifespan) in good health, free from serious disease or disability. Every evaluation is designed to protect physical performance, metabolic resilience, and long-term independence, not simply to add years, but to enhance the quality of those years.
PLC uses a comprehensive, preventive framework that includes advanced imaging, cardiovascular and metabolic testing, sophisticated biomarker analysis, and in-depth physician evaluation. Your personal wellness roadmap is created by board-certified physicians who help uncover early disease patterns, silent risk, and trajectory, often years before symptoms or routine screening would detect a problem.
Data is essential, but it is never used in isolation. PLC physicians interpret diagnostic findings within the full clinical context of the individual, integrating history, lifestyle, genetics, and longitudinal risk. This physician-led synthesis ensures care is thoughtful, personalized, and medically grounded, not driven by algorithms or standalone test results.
Princeton Longevity Center has 5 convenient locations including Princeton, NJ, New York City; Shelton, CT, DC metro and (coming summer 2026) Miami, FL. Each location is designed to deliver the same comprehensive, physician-led preventive care experience.

From dedicated care coordination to meaningful time with board-certified physicians, every step is designed to be seamless and personalized, eliminating the fragmentation, rushing, and administrative burden common in traditional healthcare while focusing fully on your long-term health and prevention.

Yes. Confidentiality and discretion are paramount at every stage of the PLC experience. Patient information is handled with the highest standards of privacy and professionalism. Even if your employer or third party is paying for your exam – they will never be privy to your results without your express written consent.
PLC provides clear, detailed pre-visit instructions well in advance of your appointment to ensure accuracy, efficiency, and the highest clinical value from your evaluation. Preparation typically includes completing a three-day food diary and vitamin log, and an extensive medical history form. Patients are also asked to fast from midnight before the exam and to avoid caffeine, vigorous exercise, or anything that may elevate heart rate for 12 hours prior to arrival. If you elect a Virtual Colonoscopy, a liquid diet and oral laxative preparation are required the day prior. Our Patient Care Coordinators are available and happy to guide you through each step and answer your questions to ensure you are ready and prepared for your day.
Results are reviewed promptly and discussed in clinical context following your evaluation. Princeton Longevity Center provides same-day results where possible, as part of every exam, including laboratory findings, imaging results, consultations, and personalized recommendations.
Occasionally, a test will be added on or is required to be sent out causing delay in results. In such cases a follow up call is scheduled with our clinical manager to review your final results.
Patients leave with a clear, prioritized plan to guide next steps in preventive care, along with copies of their test results and imaging studies to share with their existing medical providers. With same-day insight and guidance, patients can begin implementing preventive and lifestyle strategies right away.
